🌸Intensive In-Home

     -Program specific to assist parents of children who are having "life" difficulties. It helps support the child/adolescent in meeting their goals and becoming a productive member of society.  

Blossom Community Service provides highly Quality , therapeutic, and clincal services, and education geared to improve the level of functioning for recipients. We incorporate a “whatever it takes” and “never give up” attitude to providing hope, help and support.

🌸Alternative Family Living (AFL) programs provide individuals with intellectual and developmental disabilities the option to reside in a family environment. AFL programs offer integration into a family structure and community participation, which fosters a sense of belonging and independence. These programs may encompass support for daily living, social activities, and access to education, employment, and various community events.

🌸Community Networking, Community Networking services provide individualized day activities that support the waiver beneficiary’s definition of a meaningful day in an integrated community setting, with persons who are not disabled. If the beneficiary requires paid supports to participate / engage once connected with the activity, Community Networking can be used to refer and link the beneficiary. This service is provided separate and apart from the beneficiary’s primary private residence, other residential living arrangement, and/or the home of a service provider.

 🌸Supported Living provides a flexible partnership that enables a NC Innovations beneficiary to live in their own home with support from an agency that provides individualized assistance in a home that is under the control and responsibility of the beneficiary. The service includes direct assistance as needed with activities of daily living, household chores essential to the health and safety of the beneficiary, budget management, attending appointments, and interpersonal and social skills building to enable the beneficiary to live in a home in the community.

🌸Peer Support Peer Support Services (PSS) are an evidenced-based mental health model of care that provides community-based recovery services directly to a Medicaid-eligible adult beneficiary diagnosed with a mental health or substance use disorder. PSS provides structured, scheduled services that promote recovery, self-determination, self-advocacy, engagement in self-care and wellness and enhancement of community living skills of beneficiaries. PSS services are directly provided by Certified Peer Support Specialists (CPSS) who have self-identified as a person(s) in recovery from a mental health or substance use disorder. PSS can be provided in combination with other approved mental health or substance use services or as an independent service. Due to the high prevalence of beneficiaries with co-occurring disorders (mental health, substance use or physical health disorders) it is a priority that integrated treatment be available to these beneficiaries.
